Consultations are conducted by a trained nutrition professional and recommendations are made based on the principles of human clinical nutrition from a functional medicine perspective. Every client will receive personalized care and recommendations based on their health status and personal goals. Success in our program may require the client to adopt significant changes to diet, lifestyle, stress management, and exercise practices.
New Client Consultations
The New Client Package includes 4 sessions – one initial 90-minute session and two 30-minute follow-up sessions. Dawn spends a significant amount of time before the first session formulating initial recommendations based on the intake form, health records, and lab work provided by the client. The first session will include initial recommendations and any clarification needed for the final report, which will be sent to the client after the initial session.
Your initial report will include a review of findings, short and long-term goals, recommendations for diet, nutrition, sleep/restoration, exercise, stress management, supplements, goal setting, and any handouts such as recipes or dietary plans.
Follow-up Consultations
Follow-up consultations are a critical component to client success. Dietary and lifestyle changes require motivation, time, and support so that these changes can be adopted for a lifetime. We also use follow-up appointments to fine-tune and change your treatment plan over time as you continue along your journey to optimal health.
After the initial new client package is completed, established clients can choose from 30, 45, or 60-minute appointments depending on their needs.
Consultation Types
At this time, appointments are only available via HIPAA compliant telehealth video-conferencing and telephone.
You will be required to have a laptop, computer, smart phone or a tablet as well as internet access in order to participate in telehealth consultations.
Hours of Operation
Hours may vary from week to week, but generally the nutritionist is available for evening and weekend telehealth appointments. For the most current availability, see the online scheduling page.
Payment
Payment is due in full when the service is booked. We accept Mastercard, Visa and PayPal Credit. Some HSA/FSA credit cards may be accepted for services, but it is the client’s responsibility to determine if your HSA/FSA will cover nutrition consultations.
Cancellations/missed appointments
Nutrition counseling using a functional medicine approach is very time intensive. Our office requires a 48-hour notice for cancellation of an appointment. If cancelling on the same day of the appointment, but before the appointment, clients will be charged a fee of $75. No-shows will be charged the full out-of-pocket fee for their appointment. This is to ensure that access to care is not taken away from other clients and to discourage misuse of clinic resources. The fee will be deducted from the credit card on file.
Email or Messaging in-between appointments
I will generally respond to emails or messages within 24-48 hours. If the question is more in depth or takes longer than 15 minutes to answer, you will be billed for a 15 minute session for a fee of $30.
Refunds
Refunds will not be given for pre-paid plans and packages.
Medical Records
In compliance with HIPAA, your medical records can only be released with your written consent. You are responsible for obtaining your medical records for your appointment. You will be provided with a release form to use once you book your first appointment. Your records should be mailed or faxed at least one week prior to your first appointment.
Termination of Service
While this is rare, Dawn Schenone at The Functional Nutritionist, LLC has the right to terminate services with a 30-day written notice. In this case, a refund will be given for the remaining visits owed within 30 days of notification. As a client, you have the right to terminate services at any time and agree to give proper notification. See cancellation and refund policy.
Intake Forms
Our intake forms are long and detailed and will take some time to complete. These forms can be filled out online using the patient portal. It’s important that you answer the questions with detail and accuracy since this is the basis of your Functional Nutrition Plan.
You will also be asked to complete a 7-day diet diary. Please eat as you normally do during this time.
All intake forms, the diet diary, and submitted medical records/labs must be completed or received at least one week prior to your first appointment. If this is not done, your appointment may need to be rescheduled to a later date.
Financial Policy
Full payment is due prior to the first appointment.
As a functional nutritionist, I’m very passionate about helping others optimize their health through nutrition while providing the highest quality and most up to date research-based services as possible.
